Assistant Store Manager

Retail Assistant Store Manager for Hospice Partners Hope Chest, the premier thrift store in San Luis Obispo. Minimum 6 years of retail experience, at least 2 of which as a manager. Proven merchandising skills and knowledge of soft and hard goods values a must. Position will be 32 hours per week (4 days) with benefits available. High School diploma required. RN Case Manager, F/T

RN Case Managers are responsible for compassionate end-of-life care which conforms to the Hospice patients’/families’ wishes.  Visits are in patients’ homes, assisted living facilities, and skilled nursing facilities. Travel within San Luis Obispo County and Northern Santa Barbara County is required.  Two years RN experience required.  Hospice/Home health experience preferred. 

Home Health Aide Full-time & Part-time positions
This licensed position performs visits and delivers personal, practical care for Hospice patients in their homes. The Hospice Home Health Aide also provides assistance with activities of daily living and some homemaking duties.  HHA certificate is required.

RN, FT

Full time RN needed to work in skilled nursing facilities providing compassionate end of life care which conforms to Hospice patients’/families’ wishes. Travel within San Luis Obispo County and Northern Santa Barbara County is required. Two years RN experience required. Hospice/SNF experience preferred.

MEDICAL SOCIAL WORKER,  Per Diem
We are seeking a MSW who can provide compassionate social work services to Hospice patients and their families in a variety of “home” settings.  The MSW is responsible for counseling patients/families on end-of-life issues, assisting with community resources, and works with the interdisciplinary team. 
A Master’s degree in Social Work and one year of social work experience in Hospice or home health setting preferred. Position is part-time with Saturdays required.
